The holes through the ball make a huge difference, my partner can still breathe and make noise instead of panicking. Size is just right too, big enough to matter but not so huge it's uncomfortable after a few minutes.

I was worried it'd feel too big, but it's sized just right and doesn't overwhelm the jaw. Being able to still gasp and whimper through the punctured ball is honestly what makes it work for us.
